Common Agency Requirements
Preceptor agencies require students to comply with a variety of health and training requirements prior to the start of an internship. We have listed several common resources here for your convenience. Students should carefully review the following guidelines as they begin preparing for an internship.
There may be additional Compliance Requirements. All of these may not be required by the agency, or additional requirements may be needed. Check with your internship program coordinator, Julie Howell (Julie.Howell@charlotte.edu).
Please complete the Agency Compliance Requirement Form by recording your immunization dates and submitting proof of requirements. Once completed, please upload the form and copies of your requirements to the secure Dropbox link that will be provided to you by the Compliance Coordinator. If you have any questions, please contact the Compliance Coordinator at firstname.lastname@example.org *Always make a copy for your records before submitting to the Compliance Coordinator.*
